Serving food without gloves; stomach laying on platter where tomatoes were stored when reaching up for hamburgers, back door open, roach on ticket and waitress just "flipped" it off.

1.Observed the back door open and lacking a screen. 2.Observed bare hand contact with lettuce. 3.Observed soil wiping cloths lying on prep cooler. 4.Observed employee wiping out and rinsing a pan in the three compartment sink. 5.Observed menu items being added after opening. 6.Observed a rack of eggs stored next to the grill at 110F. 1.Corrected-Back door closed. 2.Repeat-Corrected-Facility placed on a risk control plan and an enforcement notice issued. 3.Corrected-Wiping cloths must be stored in sanitizing solution. 4.Corrected-All dishes must be washed, rinsed, and sanitized. 5.Contact Health Department prior to adding menu items. 6.Corrected-Eggs voluntarily discarded. 5.Observed menu items being added after opening. 6.Observed a rack of eggs stored next to the grill at 110F.
