Complaint called in that 3 people ate meatlover's pizza from pizza hut on friday night and was diagnosed at hospital with Salmonella. Symptoms started Saturday afternoon. Complaint was later changed that pizza was consumed on Thursday night (10/16/08) and symptoms started Friday (10/17/08). Pizza was a carry-out, not dine-in.

Spoke to manager about complaint, asked if anyone worked Friday night with symptoms. 1 employee had nausea, vomiting, and diarrhea and was out earlier in the week, but was diagnosed with vertigo and was back at work. Another employee that worked Friday night called in Sat. because she was sick. I asked manager about sick worker policy and he knew symptoms to look for. Dishwasher at 0ppm chlorine. Conducted another inspection on 10/21/08 (consultation). Spoke to other manager about sick worker policy and gave example forms. The 2 employees of concern did not work on Thursday night when the pizza was actually ordered. Manager said none of the employees that worked Thurs. were sick that she knew of. Discussed hygiene thoroughly and checked dishwasher again which was now 100ppm chlorine.
