WENT TO FAC FOR INSPECTION. FOUND THE FOLLOWING ON 12/15/08: 9. OBSERVED NO OPERATIONAL HANDSINK. 21. OBSERVED MAYO (OPENED), CUT TOMATOE, CUT LETTUCE, AND SLICED ONION HOLDING AT 61 F. MAYO WAS IN PREP COOLER. 2. OBSERVED NO FOOD SAFETY MGR COURSE CERT. 7. OBSERVED NO HANDSINK- NO PROPER HANDWASHING. RETURNED 12/16/08 AND ALL CRITICALS WERE CORRECTED. SPOKE WITH REG MGR ABOUT COMPLAINT AND CRITICAL ISSUES.
