Recieved complaint that person ate at the Checkers on hwy 90 in OS about 4:00pm 8/14/2012. They ate a hotdog with chili and mustard. Approximately about 1:00am 8/15/2012 the complaintant suffered vomiting, diarrhea, fever of100.3, chills,and abdominal pain.

Responded to FBI complaint. did full inspection *Beef Hotdogs - Made for Checkers/Rally in Tampa FL. 33607 Hotdogs are recieved frozen. they are thawed in walk in cooler and heated on grill then placed in hot hold apparatus. *Chili sauce is a made for Checkers product recieved in vacume pack. They use old (over 25 mins old) hamburger patties that are pulled from hot hold chopped up and mixed in chili sauce then heated to 165F using a digital thermometer to check temp. * Mustard- a made for Checkers/ Rallys product recieved in vacume pack. * Buns are a product from New Bakery made for Checkers. They are received frozen and kept until needed. They are pulled and thawed. The buns were stored on a wall rack or in the bread trays 6" off floor. * sanitizer used is Quat sanitizer and registered at 200-300 ppm Quat. * Hot hold temps- hot dogs 151F, chili 171F, & 171F * cold holding temps- 2 pans of hotdogs in walk in at 40F each Observations Observed one handsink by mopsink disconnected from wall- corrected- mgr was able to reconnect and called for maintenance. No follow up at this time
