ATE LUNCH AT CHECKERS IN OS 07/19/13 AROUND 12:00 TODAY...APPROX 2.5 TO 3 HOURS LATER STARTED HAVING SYMPTOMS OF FOOD POISONING...SWEATS, WEAKNESS, THROWING-UP...COWORKER CALLED THIS IN, WES BURKHARDT, STATED THAT MR. GRATER WAS TOOK WEAK TO TAKE TO DOCTOR AND THE TIME THAT HE WAS CALLING, BUT AS SOON AS HE WAS STRONG ENOUGH HE WOULD TAKE HIM...STATED THAT MR. GRATER STATED THAT THE HAMBURGER THAT HE ATE HAD A BAD TASTE....

Interviewed complaintant over phone and found he had eaten a Fry Lovers Burger and that once he began feeling symptoms they lasted about 2 hrs in duration and were self limiting. Complaintant di go to ER. Did inspections of facility and Mgr on duty walked through the process of the Fry lover burger from time food delivered to service. Hamburger is recieved frozen and cooked from frozen state. It is hot held no more than 30 mins. Fry lover burger is made to order. Bun is toasted and then mayo and ketchup is placed on buns, meat, cheese, mustard and topped with fries. Burger is then wrapped and served. Hot hold hamburgers 178F and 183F Walk in cooler hotdogs 41F, cheese 41F, chili (cooling) 78F line sanitizer 100 PPM No follow up at this time.
