This form is to refer an individual in need of a crib to the MSDH Safe Sleep Education and Crib Distribution Project. To receive a crib through this project, you must:

Complete this referral form
Be a Mississippi resident
Be pregnant (32 weeks or more gestation) or caring for an infant (newborn to 364 days)
Need a crib and not have another way to get one
Be willing to have a face-to-face safe sleep education encounter
Sign a Hold Harmless Agreement

Once this form is submitted, a MSDH Safe Sleep Ambassador will call to schedule your safe sleep education and crib distribution appointment. You must have a scheduled appointment to receive a crib, we cannot support walk-in requests at this time.
