open pipe leads to ditch

Investigation History
Went to site with Bill Rogers on 4/3/14. Found where installer had put in a overland discharge system at site, but was not using system yet. There was no discharge from pipe on new system. We did not see any sewage going to ditch from old system. We did notice that when new system is operational it will run to the ditch from the discharge pipe. There was no notice of intent done for this property nor any final approval filed from any installers on this property.
04-07-2015 I went to property and observed discharge from this system pooling and traveling toward the ditch via the area cut and cleared when system was installed. Photos taken. Lisa Taylor.
04-10-2015 Richard Sanford called me regarding this system. He insists he was only repairing a failing system. He further stated he asked Bill Rogers to look at it an Bill told him he couldn't with the owner completing a NOI. He wanted repair or correction advice and I suggested he do nothing further until Kevin Traylor looks at the site.
This spot was turned over to Lisa Taylor pending issues with installer(Richard Sanford).
