Investigation History  On the above annotated date Bill Roger and Anthony Claytor arrived on the site to conduct a soil and site evaluation. Upon walking around to the rear both PHEs notice that a system was installed out on the property. No recommendation was ever issued. PHE called the number listed on the NOI and spoke with Yolanda the mother of Mrs, brown. She stated that earl sanford installed the system and that the mobile home company of southern colonel mobile homes Danny smith contracted Sanford plumbing of earl Sanford. PHE called Ben Kelly to inform him of the situation.

Bill Rogers called and spoke with earl Sanford he stated that he put the system in. Informed by bill Rogers that no recommendation was issued for this site. He stated that he installed 1000 GPD tank and 100 ft of EPS. The installation occurred in the back of the mobile home. Soil boring conducted in that area soils displayed A0-8 vfsl 10yr 5/3 Bt 8-28 HL 10yr 6/6 5yr 5/8 mottle BX 28 HL 10yr 6/8 10yr 8/2. Slope in the rear at 8% approximately moving parallel with the mobile home if facing perpendicular slope in excess of 15%.

PHE Antony Claytor did another boring in the front of the property soils were far better up front. The system would need to be pumped to front and a Dbox placed with bed form products installed. Soil profile on the upland side will be on the profile page of the main file. No recommendation issued until PHE talks with regional supervisor Ben Kelly.

11/16/2016: Per Jim Weston and Regional Ben Kelly issued out recommendation. PHE Anthony claytor did put in the notes that this recommendation was done after a system was already placed into the ground. specifics of where new system are in the notes section of the recommendation. PHE also informed the mother of Mrs. Brown to make sure that she minimizes the Water useage out this dwelling until a resolution can be done. Informed that she might want to wash clothes elsewhere and keep minimal usage of the system. Case pending hearing in Jackson possibility, both PHE's Anthony clatyor and Bill Rogers awaiting update on what action will occur.

11/17/2016
PHE sent email to Ben Kelly regarding if to keep site open on list or close it out. Per the regional informed to close out the file since the division of onsite wastewater is going to handle the enforcement action on the installer and for hearing to be determined at a later date.
