Sewage system leaking in yard.

Investigation History
Observed the following:
-Cajun Aire ATU system installed three feet out of the ground.
-System installed without MSDH Form 335.
-Property owner stated system was only 2 1/2 years old.
-System leaking from the top and sides of the tank.
Requested the property owner to send me a timeline of events that has happen since the system has been installed and a copy of the manufacturer's warranty. Will then refer the complaint to the Division of On-site Wastewater.
10-24-05 Received detail complaint via email from Susan Jenkins.
10-25-05 Was given permission from Anne Hogue to send enforcement notice to Mr. Sullivan.
11-10-05 Waste water hearing. Case was resolved with a warning.
02/16/06 Received call from Jim Weston that J W Sullivan was sealing the tank and to go observed to make sure it is being sealed properly. Tank was dugged and re-sealed around seam of tank. Left property at 4:30. The tank's outlet, inlet, and lids must all be sealed and inspected by Eugene Herring tomorrow. Mr. Sullivan was told not to cover tank until Mr. Herring inspected the tank.
02/17/06 Eugene Herring inspected tank.
03/10/06 Returned phone message to Mrs. Jenkins. She said that the tank is still leaking. Called Eugne Herring, was requested by him to inspect the tank. Observed sewage on the surface by the chlorinator. Called Mr. Herring back, reported what I observed said he will speak to Jim Weston on Wednesday, the 15th to discussed what action to take next. Called Mrs. Jenkins and told her we have to consult with Jim Weston.
5-31-06 Memo (on file) from Jim Weston stated that tank was still leaking due faulty workmanship.
6-12-06 J. W. Sullivan request that spray heads be laid out on property for spray system.
6-27-06 (J. Richardson) Inspected installation of spray tank & spray field. Both tanks and chlorinator tube were properly sealed. 3-23' heads installed.
