Complaint  Wastewater at surface.
Date Of Resolution  10/27/2010
Investigation History  9/7/10- Received a call from licensed installer John Seldon. John said a store owner was complaining that a system he installed in September of 2009 was failing and wastewater was running down into a parking lot and the smell was terrible. John said the storeowner requested that he remove the ATU that he had there, and replace it with a septic tank and fill lines. John said he was told by someone at the Marshall County Health Department, unclear who told him, to replace the ATU with septic and field line. John said he installed a 1000 gallon septic tank and 250 foot of 8 inch sock pipe.

Sam (storeowner) said he paid John $6000 dollars(he has receipt) for a system that didn't last 1 year.
Supervisor Romana Reed involved, she wants John to replace system or face disciplineary action i.e. license revoked.

9/21/10- Spoke to John Seldon today. He said he was contacted btystoreowner to replace existing ATU. He said he had a contract with storeowner to remove old ATU and install new septic tank and field line. John said he called Health Department and talked to me. He said he told me he wanted to do a repair and that I said to go ahead, that I didn't need to be involved because the repair didn't require a final approval. I know I never told him that, I don't remember ever talking to John Seldon about any other job except the one at his own home.
I ask John what he was willing to do to rectify the situation and he said that he could not afford to do anything since building was slow and that a recent court case had gone against him. I said that I thought he should replace the septic and field lines with an ATU pumped to overland discharge. He said he didn't think it was his place because he said he explained to the storeowner that going away from the ATU was a bad idea in the first place.

10/4/10- disiplinary letter mailed to John Seldon

10/27/10- Licensed InstallerBrent Carpenter installed a 750 gallon per day H-TWO-O Series 2 ATU with 1000 gallon pretreatment pumped to overland discharge.
