I visited this property as requested by the owner to evulate this system. Upon arrival I observed the above installer had installed a disposal system with a recommendation. He did not contact this office to inform an inspector that he would be installing a system. Once I gave him a recommendation and the system was finsihed he did not submit an affidavit of installation.

An enfrocement notice was issued for the above violation. Within 5 working days MR Phillip had returned all nessary paperwork in accordance with Waste water regulation.
